Dr. Jonathan Falanga has left the office to begin his own practice: Serenity Chiropractic Health in Penfield, NY. He invites everyone to visit if they're in the area.

Patient forms are now available online! Simply go to the Patient Forms link at the bottom of this page and click on the text link corresponding to the forms you need to begin the download. This gives patients the option to fill out the forms at home at their own pace, instead of while sitting in the office.

Sorry, we are not accepting Aetna insurance plans at this time. We still participate with most BlueCross and many United Health Care and POMCO plans.
